Living with Heart Failure at Home The Living with Heart Failure at Home CHF program is specifically designed to meet the needs of the congestive heart failure patient. These patients may be experiencing:


  • • Unstable CHF
  • • Frequent exacerbations of CHF
  • • Advanced stages of CHF

 

With our Interpath specialized pathways; patient teaching; CHF patient teaching home videos, and Honeywell HomMed monitoring devices, we can provide care for the CHF patient to prevent exacerbation and frequent visits to the Hospital or Emergency Room.


 

Telemonitoring The Honeywell HomMed telemonitoring program provides for daily vital signs management for patients who are experiencing unstable conditions such as:


  • • CHF
  • • Diabetes
  • • COPD
  • • CAD

By utilizing the home monitoring device, we can provide vital signs management 7 days a week in a preventative and proactive manner and manage trends in vital signs measurements.

 


Hip and Knee Joint Replacement Program The Hip and Knee Joint Replacement program is an integrated approach for the home health care needs of the joint replacement patients using our care paths and teaching materials. We partner with the orthopedic surgeon to integrate the Physicians preferences for therapy and post-surgical care. Our post surgical care includes:


  • • Physical therapy
  • • Wound care
  • • Medication management
  • • PT/INR
  • • Pain management
  • • Safety management

 

Our pre-surgical videos provide patient education on pre-surgical and postsurgical expectations. Our joint replacement program is designed to improve patient outcomes and satisfaction.


 

Sure Steps Fall Prevention Program Sure Steps Fall Prevention program is a comprehensive program that addresses one of the greatest issues facing elderly Americans-falls. The program is designed to:


  • • Assess and evaluate factors that increase the risk for falls by
       using our Fall Risk Analysis Tool and Home Safety Checklist
  • • Educate the client and family about falls and means to reduce the risk for falls
  • • Communicate with the Physician, the patient, and the family regarding the risk for falls

 

Risk reduction strategies include review of the home environment, the disease process, physical strength and agility and medications.


 

Alzheimers and Dementia Care Program The Alzheimers and Dementia Care program is an exclusive partnership with the Alzheimers Foundation of America (AFA) that takes a comprehensive approach to caring for individuals with Alzheimers and Dementia as well as their families. We provide:


  • • Advanced training for professionals caring for individuals with Alzheimers and Dementia
  • • At home assessment
  • • Education for individuals with Alzheimers and Dementia as well as their caregivers and
       family members
  • • Communication with the individuals, caregivers, family members, and Physicians when
       appropriate

Good for the Soles Foot Care Program The Good for the Soles Foot Care program is a comprehensive program designed to manage foot care needs of patients with wounds, diabetes or peripheral neuropathy. Our program includes the following:


  • • Observation and assessment for early detection of foot ailments and systemic diseases
  • • Teaching on the care of the feet
  • • Complete foot care for patients who are unable to perform self foot care
  • • Monofilament sensory testing
  • • Footwear evaluation
  • • Toenail trimming
  • • Corn and callous care
  • • Foot and nail hygiene
  • • Wound care

Anodyne Therapy The Anodyne Therapy System is an FDA-approved infrared medical device that increases circulation and decreases pain. The Anodyne Therapy System may be used as a part of a comprehensive treatment plan for patients experiencing numbness, tingling, loss of sensation and pain.

Living with Diabetes Our “Living with Diabetes at Home Program” is a Chronic Care Solutions Program designed to address the special needs of the Diabetic patient. Our Chronic Care Solutions program is designed to:


  • • Educate the patient on the disease process
  • • Teach the patient how to monitor their blood sugar levels
  • • Teach the patient on diet and lifestyle modifications
  • • Teach the patient on medications, including insulin administration
  • • Instruct on signs and symptoms of hyperglycemia/hypoglycemia
  • • Manage foot care
  • • Monitor for early detection of diabetic complications

The “Living with Diabetes at Home Program” meets the unique needs of the Diabetic patient. Patient education is designed to provide the Diabetic patient with lifetime skills and knowledge to manage their Diabetes.
